Developed by physicians and physiotherapists, the app includes a Beginner Program, Advanced Program, Speed Program, Cervical Spine Program (neck), and Lumbar Column Program (lower back). The app features a video demonstration of each exercise with optional timer function, 3 educational videos, an exercise calendar, and tips on specific problems. Each program is designed to take approximately 30 minutes, and is performed 2 or 3 times per week. The Speed Program, to be used only when time does not permit a normal program, can be completed in just 10 minutes. BackDoctor 1.0 for iOS, with exercises for a healthy back and spine, is also available in an HD version for iPad.

Operable in either portrait or landscape mode, the app starts with a full screen view of the human spine and back. The screen shows which of the 5 programs the user last used, the last exercise completed, and when they are scheduled to exercise next. Control buttons along the bottom of the screen include: Home, Programs, Calendar, Basics, and More. The Programs button displays a scrollable screen of the 5 different programs, an Introduction, and an All selection that allows access to all 5 different programs without returning to the Programs page. The Beginner Program consists of 10 basic exercises, each with its own page that includes photos and instructions. From the top of any page the user may activate an exercise timer, or play back the video demonstrating and describing the exercise.
The remaining 4 programs also have 10 exercises each, and the Speed Program contains 7 quickly completed exercises. Both the written instructions and the video narration provide complete guidance regarding stance, movement, alignment, and duration of each activity. Touching the Calendar button brings up an exercise calendar that the user can use to schedule sessions and keep track of their progress. The Basics button accesses 3 animated videos: Anatomy - The Structure of the Spine; Trunk Muscles - Significance of Your Trunk Muscles; and Disc Herniation - How It Happens. Finally, the More button reveals the Setup page where various settings can be set on/off, including push notifications for the next exercise period.
